Credit Card Authorization Form

I   client of Chicago Hajj & Umrah Group, is to confirm that, in keeping with all applicable laws. We are instructing to Chicago Hajj & Umrah Group to issue the Airline Tickets and Hotel packages as per the credit card detail listed below.

I furthermore agree to pay any Panalties for Airline or Hotel Packages in case I re-issue or cancel my ticket or package. In addition to both ticket and package services, I also authorize to Chicago Hajj & Umrah Group to change my ticket and package fees as per Airline or Hotel Package charges.

Being a card holder, I understand and accept the Airlines restrictions and I also accept that most of the Airlines tickets are Non-Refundable.

    I agree and commit that I will follow all the rules for Cancellation & Date Change, and also the Terms and Conditions for Airlines and Hotels.

Please email us copy of your Credit Card both side and also copy of your ID card or Driver licenses in order to book your ticket(s)

Please Note: If the Saudi Embassy is unable to provide visa on time, Chicago Hajj & Umrah will not be responsible for any damage/cancelations. If this occurs, try and keep a positive mind set as ticket/hotel booking can be rescheduled with an additional charge.
